Pain Reprocessing Therapy (PRT) is an evidence-based approach that treats chronic pain as a learned, reversible nervous system response rather than purely a structural or tissue problem. Most PRT providers are mental health provieders or coaches — but pain also responds to how we move, and that's where physical therapy fits in. As a licensed physical therapist trained in PRT, I combine both: helping you understand why your nervous system is generating pain, while guiding you back into movement your body has learned to fear.

Your brain has learned to perceive normal movement as a threat, sending pain signals to protect you even when there is no actual injury. Can a physical therapist do Pain Reprocessing Therapy?

While Pain Reprocessing Therapy has traditionally been delivered by mental health providers, physical therapists are uniquely positioned to apply it — we understand movement mechanics in a way most PRT providers don't. I combine PRT's cognitive tools with graded movement exposure to help you physically reclaim activities your body has learned to protect against. In 15 years as a PT, I've seen many patients with nearly every kind of "abnormal" finding — poor posture, disc bulges, arthritis, asymmetries — who had no pain at all. That pattern, seen again and again, is part of what makes it possible to reduce the fear around your own diagnosis or scan results: these findings are common in pain-free people too, and they don't have to define what your body is capable of.

Is PRT the same as "it's all in your head"?

Not at all. Your pain is 100% real and physically experienced in the body. However, the source isn't always structural damage; often it is a "false alarm" from a sensitive nervous system. PRT treats the very real brain-body connection that generates those signals.

Do you treat nervous system dysregulation or central sensitization?

Yes, this is my primary focus. These are terms for a nervous system that has become stuck in a state of high alert. My approach uses somatic tracking, graded exposure, and education to calm these sensitized pathways, moving you from a "threat" state back into "safety."

Is this covered by insurance or Medicare?

I accept Medicare and offer out-of-network billing support for PPO/POS plans for telehealth physical therapy in New York and New Jersey. I provide a detailed superbill you can submit to your insurance for potential out-of-network reimbursement. Pain coaching is available worldwide but is not covered by insurance.

Do you offer this via telehealth?

Yes — all sessions are conducted via secure, HIPAA-compliant video platforms. Telehealth is especially effective for PRT and chronic pain work because it lets us work in the environment where your pain actually happens, not a clinic room disconnected from your daily life.
